IEP forms & systems
No single mandated statewide IEP system. ODE publishes standard IEP forms, and districts and education service districts choose their own IEP software.

Where Oregon differs from the federal baseline

  • The initial evaluation clock is 60 school days, measured from written consent to the eligibility meeting, not to the evaluation report (OAR 581-015-2110).
  • Reevaluations follow the same 60 school day timeline as initial evaluations.
  • Informed written consent must meet OAR 581-015-2090 and 581-015-2095 before any evaluation or reevaluation.
  • Education service districts and regional programs deliver many low-incidence services, so evaluation teams often include ESD staff.

Current monitoring priorities

  • Initial evaluation timeline compliance (Indicator 11)
  • Use of abbreviated school day programs and restraint and seclusion reporting
  • Secondary transition IEP content (Indicator 13)
